Community Care Coordination Team Leader

Care coordination is a key function within Access. The Regional Care Coordination Team Leader leads a busy care coordination team to ensure client support needs are met by trained and available support workers. This role requires sound judgement, strong planning and prioritisation, confident decision-making, and the ability to build positive relationships across Nursing, Care Coordination and Support Worker teams. Your ability to stay calm, think quickly and lead effectively in a fast-paced environment will be key to your success.

Takohanga | Responsibilities

• Working as part of the regional leadership team to deliver safe, responsive and high-quality home and community support services to our clients
• Facilitating cooperative and cohesive working relationships between Nursing, Care Coordination and Support Worker teams
• Leading a team of care coordinators to support the delivery of a responsive, client-focused home support service
• Ensuring support workers are rostered safely and effectively to meet the needs of our clients
• Overseeing all new referrals to ensure effective rosters are put in place
• Managing escalations and complaints with sound judgement, professionalism and a focus on timely resolution
• Managing team appraisals, coaching conversations and constructive feedback to support team performance and development

• A strong client focus, with the ability to balance client needs, support worker availability and service delivery requirements
• Experience in leadership or guiding others, with an understanding of how to support, motivate and coordinate a team.

• Excellent written and oral communication skills
• Thrives in a fast-paced environment
