A well-established hospice organization is seeking a full-time RN Case Manager to provide compassionate, patient-centered care in a community-based setting. This role is ideal for a nurse who values meaningful work, interdisciplinary collaboration, and balanced caseloads.
The RN Case Manager is responsible for coordinating and delivering hospice care services to assigned patients while supporting families through the end-of-life journey. This role works closely with an interdisciplinary team to ensure quality, compliance, and continuity of care.
Key Responsibilities
- Manage a caseload of approximately 10–14 patients
- Conduct patient assessments and develop individualized care plans
- Coordinate care with physicians, social workers, aides, and other team members
- Provide education, emotional support, and guidance to patients and families
- Complete timely and accurate clinical documentation
- Participate in interdisciplinary team meetings as needed
Qualifications
- Active Registered Nurse (RN) license in the state of employment
- Minimum 1–2 years of nursing experience (hospice or home health preferred)
- Strong clinical judgment and organizational skills
- Effective written and verbal communication abilities
- Ability to work independently while collaborating within a team environment
- Valid driver’s license and reliable transportation
Preferred Certifications
- Hospice and Palliative Nursing certification (preferred, not required)
- Current CPR certification
