What Will Your Day Look Like?
You'll travel throughout the St. Paul territory , providing skilled nursing visits to patients recovering or managing their health at home.
Your visits may include:
- Comprehensive nursing assessments
- Wound assessment and wound care
- Medication reconciliation and management
- Injections and other skilled nursing procedures
- Monitoring acute and chronic medical conditions
- Patient and caregiver education
- Identifying and communicating changes in condition
- Coordinating with physicians and other providers
- Collaborating with PT, OT, SLP, social work, and other members of the care team
- Helping prevent avoidable emergency room visits and hospitalizations
- Completing timely documentation in Homecare Homebase (HCHB)

Who Does Well in This Role?
You don't need someone standing next to you to know what good nursing looks like.

We're looking for:
- An active Minnesota Registered Nurse (RN) license
- Previous nursing experience in home health, hospital, rehabilitation, skilled nursing, or another clinical environment
- Strong assessment and critical-thinking skills
- Confidence working independently in the field
- Excellent patient and family communication
- Strong organization and time-management skills
- Ability to collaborate effectively with an interdisciplinary team
- Reliable transportation and willingness to complete visits throughout the St. Paul territory
Previous home health experience is valuable, but we're also interested in experienced nurses from other settings who are ready to bring their clinical skills into the home.
