As a Mental Health Professional on an Assertive Community Treatment Team or the Forensic Assertive Community Treatment Teamteam, you’ll work alongside a multidisciplinary team to bring services directly to individuals in the community. Rather than relying solely on traditional office-based treatment, you’ll have the opportunity to meet people where they are, build meaningful relationships, and help them achieve greater stability, independence, and quality of life.

Make a difference where it matters most. Join a collaborative Assertive Community Treatment (ACT/F-ACT) team providing recovery-focused, person-centered services to adults living with severe and persistent mental illness. This role offers the opportunity to provide meaningful clinical care while helping individuals build stability, independence, and connections within their communities.

What You’ll Do

  • Provide clinical treatment: Deliver individual and group therapy using evidence-based and individualized approaches, provide symptom management and psychoeducation, and support individuals in achieving their treatment goals.
  • Lead coordinated care: Develop and monitor person-centered treatment plans, coordinate services across the individual’s treatment team, and collaborate with families, community providers, and other professionals.
  • Support recovery in the community: Provide practical, rehabilitative, and skill-building support that helps individuals strengthen daily living, social, vocational, housing, and community-integration skills.
  • Respond to changing needs: Conduct ongoing assessments, monitor progress and response to treatment, provide crisis intervention when needed, and adjust services to meet changing needs.
  • Be part of a multidisciplinary team: Participate in daily team meetings, treatment planning, cross-training, and shared clinical decision-making while providing services under the guidance of the team leader and psychiatrist.
